Early this month, I got the chance to process my Yellow Card also known as the International Certificate of Vaccination from the Bureau of Quarantine. Even if you don't have international travel yet, you can still process yours. Here's a step-by-step guide for you on how to process your Yellow Card!

First, what is this Yellow Card? According to Wikipedia, The International Certificate of Vaccination or Prophylaxis (ICVP), also known as the Carte Jaune or Yellow Card, is an official vaccination report created by the World Health Organization (WHO). As a travel document, it is a kind of medical passport that is recognized internationally and may be required for entry to certain countries where there are increased health risks for travelers.

Before you proceed, I suggest you'll use a laptop or desktop. The mobile phone is okay but knowing the government website, it keeps on freezing, thus you need to restart the browser again and again. 

1. Open your preferred Internet Browser and open https://icv.boq.ph/
2. Click Select Location and Choose Visayas

3. Under Visayas, there are two options. I prefer to choose Robinsons Galleria as open slots are still green for the rest of the month.
4. Click Book an Appointment, read the items in the pop-up and click "I Agree to the Terms & Privacy..."
5. In the next pop up, choose NEW CERTIFICATE
6. Fill up the form with all information and click SUBMIT

Once all set for above, you will be shown a 5-steps guide.

Step 1: Choose BOQ Service Type | Choose International Vaccine Certification Services | Choose Certificate of Vaccine for COVID 19 | Then click NEXT
IMPORTANT: The client needs to submit an original copy of his/her COVID 19 vaccine certificate issued by the Local Government Unit (LGU). The traveler must be fully vaccinated against COVID-19 in the Philippines.

Step 2: Pick your Preferred Date to Visit - Make sure to choose the Green highlights as this means open slots. Then choose Appointment Time

Step 3: (Client Info) - You need to fill in all information which includes Documentary requirements such as Government ID (PRC ID, VOters ID, GSIS, SSS/UMID, Drivers ID, or Postal ID), VACCINATION CARD/CERTIFICATE, and Passport.

Step 4: Confirmation - You need to review all information you fill in step 3

Step 5: Payment - there are different options for how to pay such as Paymaya, Gcash, or Credit/Debit Card. Just follow accordingly. Once payment is done, you are good to go! Do not forget to save the Reference Number.

What to bring during the appointment?
Here are the important items, you need to bring.
1. Valid ID such as UMID, SSS, see above.
2. Passport (not expired)
3. Vaccination Cards issued by LGUs.

What are the steps during the appointment?
Step 1: You need to fill out a form from the reception
Step 2: You'll wait for your scheduled appointment
Step 3: Verification - You'll wait for your turn and pass your passport and vax id so that the personnel can verify your data on their system.
Step 4: Printing - The assigned personnel will proceed on printing your Yellow Card.

Does the Yellow Card have an expiration date? NO, and YES.
No, if your Passport is not yet expired.
Yes, if your passport is soon to be expired.

What does this mean? The expiration of your Yellow Card will depend on the expiration of your Passport because the ID stated in the yellow card is your passport ID.
